What Is BOCA?
BOCA is Cadmus Labs’ flagship project for developing a new approach to dental benefits.
It begins with BOCA Direct, a standalone software business for independent dental practices. Research, operating experience, and evidence will help shape the path toward BOCA Prime—the new model of dental coverage Cadmus is developing—and the technologies that may eventually support it.
If we were designing dental benefits today, what would we intentionally build differently?
That question runs through the entire project.

BOCA Prime is Cadmus Labs’ effort to give patients and dental practices a financial answer they can understand and rely on before care begins, then carry that determination into payment with less unnecessary friction and overhead cost.
Its larger aim is to keep more of what people already pay available for care and help make dental care more accessible and affordable.

Cadmus is investigating how BOCA can develop additional capabilities over time—from trusted digital identity and evidence verification to new ways of extending affordability and giving participants a greater voice in the system.
Concepts including Rootprint™, DentAI, and Rootline are being developed alongside research into blockchain, governance, and emerging financial technology to determine where each could provide meaningful value, help protect the system and its participants, and create additional opportunities for Cadmus to generate revenue.
THE PROBLEM
A Growing Disconnect
Dental benefits were created to help patients access and afford care.
For many patients, however, the relationship between what they pay and what they receive has become increasingly difficult to understand. The cost of care continues to rise, benefits often remain limited, and more of the financial responsibility falls directly on the patient.
Dental practices experience the same system from the other side: through reimbursement pressure, complex rules, and less control over the financial relationship with their patients. They also carry much of the administrative burden—checking benefits, obtaining preauthorizations, submitting and resubmitting claims, and resolving discrepancies when the final payment does not match what the practice and patient expected.
The result is not one isolated problem. It is a growing disconnect between what patients expect, what practices are asked to manage, and what the system consistently delivers.
For patients
More responsibility. Less clarity.

These pressures do not necessarily reflect bad intentions or one point of failure.
They raise a more important question:
If dental benefits were created to improve access to care, how did we get here?
THE INHERITED SYSTEM
Why This Problem Persists
The challenges facing dental benefits did not appear overnight.
They are the result of decades of incremental change, with each generation attempting to solve the problems that came before it. New rules were added. New administrative processes emerged. New stakeholders entered the system. New incentives developed.
None of those changes were necessarily unreasonable on their own.
Together, however, they created a system that has become increasingly difficult to simplify.
Over time, everyone adapted.
Patients adapted. Dental practices adapted. Employers adapted. Insurers adapted. Software adapted.
Each participant learned to operate within the system that already existed.
As a result, improvements often focused on making the existing structure work a little better rather than asking whether the structure itself should be reconsidered.
Making a system work better is not the same as asking whether it should work differently.
THE APPROACH
A Different Question
The current dental insurance system reflects decades of accumulated change, shaped by evolving assumptions, incentives, and complexity. Understanding how those forces shaped the system makes today’s outcomes easier to explain.
BOCA begins by asking what should be carried forward, what should be changed, and what could be designed differently.

If we were designing dental benefits today, what would we intentionally build differently?
That question reaches beyond administrative efficiency and opens the door to reconsidering the system more broadly: how it should work for patients and dental practices, how they should interact with it, and how their interests can remain represented as the system grows.
A system can begin with one purpose and change considerably as it grows. New financial pressures emerge. New interests have to be served. Leadership changes. Rules change. Over time, the needs of the institution itself can begin to compete with the needs of the people it was created to serve.
BOCA is being designed with that possibility in mind. Cadmus is exploring whether important protections can be built into the system itself: rules that are clear and consistently applied, information that can be verified, greater control over access to personal information, and ways for patients and dental practices to have a meaningful voice in decisions that affect them.
Emerging technologies may make those protections possible in ways that were difficult to achieve before. BOCA is investigating where blockchain, privacy-preserving technology, digital identity, and new forms of governance can provide a real advantage.
A different approach has to prove itself in practice.
THE FIRST BUSINESS
The First Step
Every ambitious project eventually faces the same question:
Where do you begin?
You begin where meaningful value can be created today.
You begin where progress can be measured.
You begin by earning the right to build something larger.
That is why BOCA begins with BOCA Direct.
Independent dental practices already offer in-office membership programs as a more direct way for practices and patients to organize payment for care outside traditional insurance. As enrollment grows, operating those programs consistently can create its own administrative burden.
BOCA Direct is being developed to carry that operational weight.
That makes Direct a meaningful business in its own right—and gives Cadmus a practical place to begin learning alongside the independent practices BOCA ultimately hopes to serve.
As practices use Direct, Cadmus can learn from real membership programs, real workflows, real patient participation, and the people operating those programs every day.
Direct is openly being developed as the first business within the larger BOCA project. Practices using it will know what Cadmus is working toward. That creates an opportunity for interested dentists to contribute their experience and perspective to Prime’s development—and potentially become part of the group of early practices willing to help test what comes next.

THE LARGER PROJECT
Building Toward Something Larger
Direct begins with work dental practices face today.
As that work progresses, experience can grow. Evidence can accumulate. Understanding can deepen.
BOCA Prime is the new model of dental coverage Cadmus Labs is working toward. It begins from the premise that a patient and dentist should be able to understand what coverage will contribute and what the patient may owe before treatment begins—and that a determination relied upon before care should carry forward when the established conditions are met, reducing the risk of financial surprises after treatment.
Prime also investigates whether the system can make better use of the money already going into dental coverage.
When a patient and dentist receive an answer before treatment, that answer should be something they can rely on. Practices should spend less time chasing claims, correcting discrepancies, and resolving problems after care has already been provided. And fewer resources should be consumed simply moving information, decisions, and payments through unnecessary layers of administration.
Cadmus is investigating whether improving those parts of the process could reduce avoidable friction and overhead—and preserve more of the money already entering dental coverage for the care it was intended to support.
Solving those problems leads to needs that extend beyond coverage administration itself.
Patients need a reliable way to establish who they are and carry trusted information with them. Evidence needs to be verified without creating another layer of manual review. Coverage alone will not make every treatment affordable. And as BOCA grows, questions of how money moves, how important decisions are made, and how patients and dental practices remain represented become increasingly important.
Those needs have shaped the larger BOCA architecture and the development of concepts including Rootprint™, DentAI, Rootline, and future financial and governance mechanisms. Each began with a problem BOCA is trying to solve and must ultimately earn its place by creating meaningful value for the system and the people participating in it.
BOCA Prime is not available today. Its legal structure, economics, technical design, governance, and final form remain subject to substantial research and real-world validation.
Experience grows. Evidence accumulates. Understanding deepens. The next step is earned.

THE DEVELOPMENT METHOD
Evidence Determines What Advances
Every significant decision within BOCA begins with a question.
Cadmus Labs publishes the reasoning and evidence behind BOCA so others can understand how the company reached its conclusions—not just the conclusions themselves. That includes the evidence supporting an idea, its limitations, the assumptions it depends on, alternative paths considered, and questions that remain unresolved.
Research does not guarantee that the current design will survive unchanged. It helps determine which ideas deserve to advance, which need to change, and which should remain unbuilt.
And no single body of research can answer every question BOCA must resolve. Legal & actuarial feasibility, economics, clinical considerations, technology, privacy, security, market acceptance, and real-world operation each have to withstand their own scrutiny.
A compelling idea earns investigation. Evidence earns implementation.
