Rethinking Dental Coverage

Dental care has changed. Dental benefits have struggled to keep pace.

The current system leaves many patients carrying more of the cost while dental practices absorb growing administrative complexity.

Too much of the value entering the system is consumed before it reaches care.

Cadmus Labs is asking how much of that value can be preserved for the purpose it was meant to serve: helping people obtain dental care.

BOCA is Cadmus Labs’ flagship project for pursuing that question—bringing software, research, and emerging technology together to move more of the system’s value closer to care and give the people receiving and delivering that care a greater say in the decisions that shape the system.

BOCA Direct

An in-office membership program can give a dental practice and its patients a simple, more direct way to pay for care. As enrollment grows, the practice must also manage the payments, renewals, benefits, missed visits, exceptions, and expectations involved in keeping the program working.

BOCA Direct is being developed to carry that operational weight, helping independent practices build stronger membership businesses while giving patients a practical option outside traditional insurance.

BOCA Prime

A patient can sit with a dentist, understand what care is needed, and still have no clear answer about what their coverage will do.

The practice can submit a request for preauthorization, provide the required evidence, and receive an indication that the procedure will be covered, and still learn after treatment that payment has been reduced or denied.

This is how the current dental insurance system can behave: it reviews the required evidence, interprets its internal rules, and provides an initial determination, then moves the final claim through its machinery—only to change that determination after the care has already been delivered and the costs have become real for both patient and practice.

The answer arrives too late.

Dental coverage should be understandable before care begins—and do more with what people already pay.

BOCA Prime is Cadmus Labs’ effort to build a better way for dental coverage to work.

The idea begins with something simple: when a patient and dentist are deciding on care, they should be able to understand what the coverage will contribute and what the patient may owe—before treatment begins. And when the system gives them that answer, they should be able to rely on it.

Prime would bring together existing technology with new systems being developed by Cadmus Labs to make that possible. It could confirm that the patient is covered, identify and verify the evidence needed for the proposed care, and apply established coverage rules consistently to determine what Prime will contribute and what the patient may owe. Those rules can evolve, but changes should be clear and traceable rather than quietly changing an answer after a patient and dentist have already relied on it.

Once the required conditions are met and the care is provided as approved, that earlier determination could carry forward into payment and settlement rather than beginning another round of review that can produce a different answer.

Cadmus is also investigating a larger question: if more of this process can be coordinated clearly, verified efficiently, and settled with less unnecessary friction, can more of the money already going into dental coverage ultimately be available for dental care?

Direct begins with work dental practices face today. Research, operating experience, and evidence will continue to shape the larger Prime model as Cadmus works toward it.
